- log-in to InkSruvey
- click on "add new question"
- add title and question in the appropriate boxes
- put a check mark in the boxes below to have only graphical response or only text response or both
- put a check mark in the boxes below to require an identifier or ask for a confidence rating
- click create question which sends you to a page confirming the creation of the question
- click admin page
