How to generate an InkSurvey account

1. Go to ticc.mines.edu
2. click on InkSurvey Tool
3. click on create account (on the upper left of this web page)
4. add the requested information and then click on submit
5. go to your email account (used in your submission above) and click on the link to active the account
6. you will receive another email with the web address for your InkSurvey web page
